Upload a File

This is a secure file upload to send materials directly to the Fremont Lake Union event approval board. Uploaded files will not be publicly visible.

All applications require the following documents:

  1. Certificate of Insurance with a minimum of $2,000,000.00 naming ‘Quadrant Lake Union Owners Association’ as the additional insured for the event. If submitting applications for multiple events throughout the year please provide one Certificate of Insurance covering all event dates.
  2. If alcohol will be served or allowed outside, increased insurance will be required at a minimum of $3,000,000.00.
  3. Certificates of Insurance for each event vendor
  4. Any additional documents

Documents must be uploaded individually. Form will reset to allow for an additional upload after each submission.

Please title all files in the format: Event Name_Company Name_Document Title.Doc Type
